Health at Home
At Advocate Aurora Health, we understand that recovering from an injury or illness or managing a chronic condition isn’t always easy and we know that patients often recover sooner in the familiar surroundings of their home and with the support of family and loved ones. Advocate Health at Home and Aurora Health at Home team members provide compassionate medical care and services – all in the comfort, safety and convenience of the patient’s home. You’ll work as part of a larger team, but with independence to manage your own day. Supervise medications and injections, manage chronic disease, monitor vital signs, counsel on nutrition and more, in collaboration with the on-site care teams via electronic health records and other modes of communication.
Registered Nurse
As a nurse working in home health, you’ll work with patients where they feel most comfortable: their home. Whether recovering from surgery, managing a chronic condition or nearing end-of-life, you’ll be able to help manage care, educate patients and their families or other caretakers and provide the support they need.
